This printed form for Pennsylvania loyalty oath certificates was issued pursuant to the Act of General Assembly of Pennsylvania, passed on June 13, 1777. This form was completed for “Valentine Ulrich of the Northern Liberties shopkeeper” and signed in manuscript by James Young. The form was later backed with fragments of the Pennsylvania Packet dated 1797.
