[Hezeiah] Sabin, signed Capt. Moses Gilbert and Lieut. Moses Ford.|The 4th, dated Nov. 2, 1780 is Surgeon Eneas Munson's affadavit, certifying Bassett's wound "through the chest", a "disability ... likely to attend him during life."|Watermark: GR beneath a crown
