Authorship possibly attributed to Henry Latimer who served as a hospital surgeon in the Continental Army & eventually attached to the newly created "Flying Hospital," a mobile surgical unit of the Continental Army.

Divided into 2 sections; 1st section listing more than 800 names of soldiers, organized by regiment, who were admitted to the hospital for treatment on September 16, 1778; 2nd section also organized by regiment with names, date of admission, company, diseases, and discharge (fit for duty, desertion or death) for some 679 soldiers admitted for treatment between November 1778 and early January 1779.
