On verso: I do swair that the within muster roll is a true state of the company ... to the best of my knowledge [signed?] Jas. McCulloch, Lt. McCulloch was killed July 15, 1782.
On verso: Proof of effectives
Enumerates in six columns 2 sergeants, 2 corporals, 2 drum & fife, and 27 privates, of whom are a few included remarks such as: on duty, on com'd at Paramus, with Gl. Wayne, on furlow, sick, killed, or transfer'd
