Partially printed document completed in ink in an unidentified hand, perhaps that of Daniel Rogers.
Other officers named are Robert Wallis, 1st lieutenant, and Jabez Farley, 2nd lieutenant, both from Ipswich.
On verso: No. 18 Exxex Co. Mass. 3 Regt.
Form split into "Training Band" and "Alarm List"; total strength, 3 officers and 118 men.
Also includes "The whole number of equipments" ... such as fire-arms, steel ram-rods, priming-wire and brushes, buck-shott, bulletts, knapsack, blanket, and canteen.
Whites counted separately from blacks, indians, and "molattoes"
